Please feel free to contact our office to give referral information to our staff, or if you would prefer, complete the Referral Form below. Please include most recent records concerning the eyes and any lab work that has been performed with the Referral Form.

If you would like to discuss a case with one of our doctors, please feel free to call our office and they will be happy to consult with you concerning the case.
Communications
We have an email account dedicated to communication on your client cases: aocreferrals@eyedvm.com. Feel free to send client files, film or other case-related attachments. This is a dedicated email for doctors only, not for general public use. Pet owners, we prefer to speak with you personally via phone.
Emergency Cases
If a case is urgent and requires immediate attention, please call us yourself and speak with one of our doctors or triage technicians. Our doctors may make recommendations for treatment until the patient can be seen. For after hours emergencies or emergencies during our procedure time, please call the emergency pager number at 214-926-1226.
Equine Referrals
Please call to discuss the case with our ophthalmologist. Equine examinations are scheduled on an individual basis.
